Forms having dynamic fields#20

Dynamic fields on forms that when someone chooses a particular drop down or check mark the form prompts them to follow a particular workflow for the rest of the form.

It would also be lovely, if a URL could be enterred if they choose an option that warrants the user having more information about their option.

This will allow the form to weed out unnecessary or unacceptable submissions
