Include *ALL* form fields, including profile fields, when sending text & email notifications that forms have been submitted#233

When we receive a form submission notification via email, and that form is tied to a profile, we have to manually go in to Aware3 and look up the profile of the submitter by name to get their contact information. Aware3 sends all the form data, but it excludes the profile data that has the submitter’s contact information on it.
